Player Registrations (2025/26)

This registration form is for players that are playing in Burscough FC Youth teams in the 2025/26 season.

Make sure you have read, understand, and agree to adhere to the Club Terms & Conditions, Club Fees, and all the other Club Policies within the Club section of our website before you start filling in the form.

1. Team Details

We need to know some details about the team(s) that the player is registering for.

Saturday Team *

Let us know which Saturday team the player is registering for (select 'None' if you don't play for one of our Saturday teams).

Sunday Team *

Let us know which Sunday team the player is registering for (select 'None' if you don't play for one of our Sunday teams).

2. Player Details

We need to know some details about the player you are registering.

Player Name *

Enter the player's first and last name.

Date of Birth *

Enter the player's date of birth in the following format: DD/MM/YYYY. New players should show their proof of age (birth certificate or passport) to their Manager.

Home Address *

Enter the player's full home address, excluding the postcode.

Postcode *

Enter the player's postcode.

Medical Conditions *

Tell us about any medical conditions (including learning difficulties) that we need to be aware of for this player. If there aren't any, type in 'None' so we know you haven't forgot to fill this in.

Player Photo *

Upload a recent head and shoulders photo of the player (it doesn't have to be a passport photo). We need this for league registrations.

3. Parent/Carer Details

We need to know some details about the player's parent/carer. This should be the main person the club will communicate with about football matters and for emergency contact purposes.

Parent/Carer Name *

Enter the parent/carer's first and last name.

Parent/Carer Date of Birth *

Enter the parent/carer's date of birth in the following format: dd/mm/yyyy. We need this information as we use the FA Whole Game System to register players with leagues, and the FA now insists that a parent/carer record is added too (for emergency contact purpose), so we need the parent/carer’s date of birth in order to do this.

Mobile Number *

Enter the parent/carer's mobile telephone number. Enter the number without spaces.

Email Address *

Enter the parent/carer's email address.

Additional Information

If you wish to give us additional information or contact details in case of emergency, enter them here.

4. Declaration

We hold onto the details on this form so we can administer our teams (e.g. to register players for our teams with leagues), and so we can get in touch with you about Burscough FC Youth related matters. Burscough FC Youth would like to share image(s) and video(s) of your child for the purposes of promoting and publicising the club’s activities.
