Academy Registrations (2024/25)

1. Training Details

We need to know some details about the training session the player is registering for.

Please let us know the school age that the child will be in from September 2021.

Let us know the date of your child's first session with us in the following format: DD/MM/YYYY. You can back-date this start date if you've been coming to the sessions for a while but haven't registered yet.

2. Player Details

We need to know some details about the player you are registering.

Player Name *

Enter the player's first and last name.

Date of Birth *

Enter the player's date of birth in the following format: DD/MM/YYYY.

Home Address *

Enter the player's full home address, including the postcode.

Medical Conditions *

Tell us about any medical conditions (including learning difficulties) that we need to be aware of for this player. If there aren't any, type in 'None' so we know you haven't forgot to fill this in.

3. Parent/Carer Details

We need to know some details about the player's parent/carer. This should be the main person the club will communicate with about football matters and for emergency contact purposes.

Enter the parent/carer's first and last name.

Mobile Number *

Enter the parent/carer's mobile telephone number. Enter the number without spaces.

Email Address *

Enter the parent/carer's email address.

4. Club Fees

We need to know the monthly subs you are paying. See our Club Fees page for more information.

Annual Registration Fee *

The Annual Registration Fee for players in our Academy training sessions is currently free of charge!

£0 Monthly Subs *

The academy training subs are paid for every month that you attend the sessions and must be paid by direct debit. If you were late registering and starting the payments, you can back-date them as needed.
